Sara Coleman Hernandez

Sara Coleman MPH, MBA 

Director, Population Health Quality 

Sara Coleman has 14 years of experience working on health care transformation efforts across government, health insurance, and health care systems. She currently serves as the Director of UCSF Population Health Quality & Performance Improvement, where she oversees a variety of federal, state, and health plan pay-for-performance programs that aim to reduce costs, improve quality of care, and enhance the patient and provider health care experience. She enjoys working collaboratively with clinic managers, staff, and providers to understand our patient data trends and implement new models of care delivery for our patients.   

Sara holds a bachelor’s degree in Psychology from the College of William & Mary, an MBA from The Johns Hopkins University Carey School of Business, and an MPH from The Johns Hopkins Bloomberg School of Public Health. Sara is originally from Virginia and spent her childhood in the Dominican Republic. When she’s not working on various population health initiatives, Sara enjoys time outdoors with her husband and two daughters.
