Irina Kaplan, RN BSN PHN is the Nurse Manager for the Care at Home program (Home-Based Geriatric Primary Care practice). She manages a team comprised of RNs, Social Workers, and Health Care Coordinators. In addition, she oversees a UCSF Skilled Nursing Facility Partnership program at SFCJL. Prior to UCSF, Irina worked as a Care Transition Program Manager at San Mateo Medical Center where she led and managed a team as well as provided a clinical oversight for all at risk-community members. Her other professional experience also includes working at a Managed Care Plan- Health Plan of San Mateo as well as 10 years as a Director of Clinical Services in Home Health industry. During her professional career, she’s worked in a variety of clinical settings, but the majority of her experience comes from working with vulnerable geriatric population. Irina holds a BSN degree from University of San Francisco.
