Medicare Annual Wellness Visits

This program helps Medicare patients schedule a Medicare Annual Wellness Visit (AWV) with a UCSF primary care provider.  If you are a UCSF primary care patient, our team may reach out to you to schedule your visit.  During your visit, your provider will discuss wellness, prevention, and possible risks to your health.

How the Program Works

Before your Annual Wellness Visit:

  • If you haven’t had a primary care visit in more than a year, a health care navigator may call you. 
  • You need to fill out a health risk assessment questionnaire as part of your Medicare AWV. This questionnaire asks you about your health and day to day activities. You can find the questionnaire in your MyChart patient portal 14 days before your appointment.
  • If your Medicare AWV is coming up and you haven’t filled out your questionnaire yet, a health care navigator might call you to help you complete it over the phone. 

What to Expect during an Annual Wellness Visit:

  • During your AWV, you will talk with your primary care provider about your health risks to make a personalized prevention plan for you.
  • Your AWV visit usually will not include a physical exam.
  • If you choose to talk about new or ongoing medical problems at your AWV, you might have to pay your usual Medicare co-pay.
  • After your visit, you’ll get information about preventive health care and a schedule of recommended screening tests and vaccinations for the next 5-10 years.
