Meg Wheeler, RN, MS
Manager, Care Transitions
Meg Wheeler is the Manager of Care Transitions Programs in the Office of Population Health at UCSF Health. In this role, she has led the design and implementation of innovative interdisciplinary transitional care models, including an award-winning hospital-wide centralized follow-up phone call program and specialized outreach for patients who test positive for COVID-19, high-risk Medicare patients, and Bundled Payment for Care Improvement patients. Meg oversaw UCSF’s Post-Acute Care Collaborative and served as Co-Principal Investigator and manager of a randomized controlled trial replicating the Naylor Transitional Care Model. She also manages the Population Health Relay Center; in this role, she has developed and optimized the centralized clinical team that addresses all subcritical radiology, microbiology, and laboratory results for UCSF Health. She earned her Bachelor of Science in Nursing degree from the University of Michigan in Ann Arbor and her Master of Science in Healthcare Administration and Leadership from the University of California in San Francisco.
