The Relay Center
The Relay Center helps make sure certain test results are shared with the right health care provider, so you get the care you need.
How the program works
Our team reviews specific test results, such as lab tests, imaging, or cultures. These results are not emergencies, but they may still need follow-up or review by your care team.
We check to see if the results have already been reviewed and addressed by a UCSF health care provider. If a result may need more follow-up, we take steps to make sure the right care team member is notified.
How we may contact you
A friendly Health Care Navigator (HCN) or Registered Nurse (RN) may reach out to you by phone if you have a radiology, microbiology, or lab result that needs to be shared with your primary care provider.
We may ask you who your primary care provider is so we can send the result to the correct office. If you do not currently have a primary care provider, our team can help you find one.
Our goal is to support your care and help ensure nothing important is missed.
