ED Discharge Coordinator Nurses

 

This program is designed to support you or your family member after you have been discharged from the Emergency Department (ED) and have transitioned home. Discharge Coordinator Registered Nurses (DCRNs) follow up on your test results, working with Emergency Medicine doctors to make changes to your treatment plan, and provide extra support to patients who need it. DCRNs can answer medical questions and help you understand and navigate the next steps in your recovery.

How the Program Works

After you leave the Emergency Department, we are alerted of results from tests that you had while you were at UCSF. We communicate with doctors and pharmacists in the Emergency Department to review the results and determine if you need to come back to the ED, if you need to stop or start a medication, or if you need to be evaluated further by your regular provider or a specialist. We may call you or send you a message through MyChart. Emergency Medicine providers also refer some patients to our team to assist with specific needs, such as calling to see how you are feeling or making sure you have the information to make a follow-up appointment.
