This program gives you extra support when you have a child with ongoing health concerns. Our team’s Health Care Navigators (HCN) will work with you, your child’s primary care provider, and their specialist to give the caring support your child needs to improve their health and wellbeing.
How the program works
What to Expect:
- We will connect with you by telephone, video visits, at a UCSF clinic, and in some cases in your home, to get to know your child and family better and understand your child’s healthcare challenges.
- We will help you figure out your child’s health goals and support you in connecting to resources to help reach those goals.
- We will talk with you on the phone or by video calls weekly to monthly to see how you and your child are progressing and find out what kind of help would best meet your needs.
- We may attend some of your child’s in-person and virtual clinic visits with you.
Our Health Care Navigators will:
- Help you understand your child’s health conditions and support you in the best ways to care for them.
- Review your child’s medicines with you, assist you in working with the pharmacy to make sure you can get the medicines, and review how to use them correctly.
- Coordinate with your child’s primary care provider and specialists to support them in working together to take care of your child.
- Connect your child and family to services and programs that help you all stay healthy, safe, and able to do daily activities.
- Coordinate resources to help make sure that your family has what they need at home, like food, transportation, and medical equipment.
- Offer support and advice to you and your family and teach you ways to manage stress and new skills for coping with challenges.
Completing the program
Your child will be in the program until the goals we agreed to work on are met, or until you are making good progress towards those goals and have a clear plan to meet them. Your Health Care Navigator will tell you the estimated date of graduation when you enroll in our program.
