This program helps people who have health concerns after they go home from the UCSF Parnassus Emergency Department (ED). Our advanced nurse specialists and social workers work with your primary care provider or specialist to give you the support you need to improve your health and wellbeing.
How the program works: Short Term Emergency Department Discharge Intervention (STEDDI)
Getting Started:
- We will call you to get to know you better and understand your healthcare challenges.
- We will explain what you need to do after your visit to the emergency room and give you support and resources to follow your doctor’s instructions.
- We will talk to you over the phone or by video calls every day, or based on your need, to help you get better.
Our Nurse Will:
- Help you understand your medical conditions and how to take care of yourself.
- Review your medicines with you, make sure you can get your medicines, and teach you how to take them correctly.
- Make sure that your primary care provider and specialists are working together to take care of you.
Our social worker will:
- Connect you to services and programs that help you stay healthy, safe, and able to do daily activities.
- Coordinate resources to help make sure that you have what you need at home, like food, transportation, and medical equipment.
- Offer support and advice to you and your family and teach you ways to handle stress and new skills for coping with challenges.
Completing the program
You can be in this program for up to two weeks, or until you are doing well with your treatment plan.
