Behavioral Health Collaborative Care

This program is for adult UCSF primary care patients with depression, anxiety, and substance abuse disorders.  We are a team of social workers and psychiatric consultants who work closely with you and your primary care team to help improve your mental well-being.    

How the Program Works

Joining the Program

  • You can ask your primary care provider and ask them to help you join our program. 
  • To join, you need to be 18 years or older, have anxiety or depression, and not already be seeing another behavioral health provider.

Getting Started and Support

  • If you decide to join, we will set up a first meeting to talk about how you are feeling.
  • You will then talk to a social worker over the computer or phone every week or every other week for about 3-4 months. 
  • We will check on how you’re doing and help manage your medicine and treatment with your primary care provider.

Completing the program

  • Our goal is to treat your symptoms until you are better.
  • After we work with you for 3-4 months, we will help connect you with longer term mental health care if you still need it.
