Why should you get asthma coaching? Having asthma can be tough. If it’s not under control, it could harm your lungs over time and make your symptoms worse later. Asthma coaching gives you special help to manage your asthma better and keep your lungs healthy.
How Do I Know if I Need Asthma Coaching?
- Do you have asthma symptoms or use your quick-relief inhaler more than two times per week (not including before exercise/sports)?
- Do you wake up at night with symptoms more than two times per month?
- Do you use up more than two quick-relief inhalers per year?
- If you answered “yes” to any of these questions, asthma coaching could really help you.
How Asthma Coaching Works
Getting Started:
- A health care navigator (HCN) will call you or message you on MyChart to tell you about our program and set up a coaching session with you.
Personalized Care:
- You will have a video call or phone call with our pharmacist who is an expert on asthma.
- Your first talk usually lasts about one hour.
- The pharmacist works with your primary care provider to pick the best medicine for you.
- You’ll have another appointment with the pharmacist about a month later to see how you’re doing. You can have more meetings with the pharmacist if needed.
What We Work On:
- Making sure your asthma is under control, so you don’t feel sick or need your quick-relief inhaler.
- Learning what makes your asthma worse and to avoid these things.
- Knowing the early signs of an asthma attack.
- Learning how to use your medicine the right way.
- Keeping your Asthma Action Plan up to date. This plan helps you know:
- When to use your daily medicine.
- When to use your quick-relief medicine.
- When to call your doctor.
- When to get emergency help.
Graduation and What’s Next:
- Once your asthma is doing well, you’ll finish the coaching program and just see your regular provider or lung specialist as needed.
- You can always reach out to us if you need help later.
